Effective 07/01/08:
- the Voluntary Paternity Acknowledgment and Voluntary Legitimation form was updated (PA form);
- Voluntary Legitimation is available up until the child’s first birthday only;
- The PA form may be completed anywhere as long as both the mother and father’s signatures are witnessed by a notary public; and
- If the PA form is completed anywhere except a hospital, state or county vital records office or probate court, both the mother and father’s signatures must be witnessed by a notary public. It is then the responsibility of the parents to provide the original document to the State Office of Vital Records for filing.
